Why Q Health Services

The Board of Directors of Southside Community Health Services felt that they needed a way to stabilize the uncertainty of funding for both their medical and dental programs, plus a vehicle with a strong focus on the development of innovative programs to reach out to the communities they serve. It was based on this need that Q Health Services, an affiliate of Southside, emerged.

The purpose of this organization is to provide community outreach through Q Health Connections, develop disease management programs based on performance measurement and quality outcomes through Q Health Clinical Intelligence, and raise funds to support these programs, and support Southside during health care market downturns, through Q Health Foundation.

Q Health Foundation is the first foundation in the state of Minnesota to be established by a Federally Qualified Health Center for the purpose of raising funds to improve community health. We are committed to getting people to become more involved in their own health care – this is accomplished by providing the resources and support to increase their health care knowledge, understand the chronic diseases that may affect them, and learn how to manage their personal health.

We have found that health disparities in Minnesota are often more a case of not having a connection with a health care provider than services not being available. Through our programs we hope to develop and nurture that connection, and to let people know that they are welcome to our organization before they have a need.

It is our vision to be the preeminent community health care model through providing quality care, community outreach and innovation in our approach to the health care market.
